Patient referrals and transfers

Referring or transferring patients to Shriners Healthcare for Children — Twin Cities is a simple process, which only involves a phone call. Any child age 18 or under is eligible for care as long as the child’s condition is within the scope of services provided. All children are treated regardless of the families' ability to pay.

Referral process

For providers

There are multiple ways to refer a patient:

  • Complete our Patient Referral Form and fax it to 612-596-6102.
  • Call our referral department at 612-596-6105.
  • Use your own form through your electronic medical records.

After we receive the referral, our referral coordinator will contact the family within 48 hours. Once the appointment date is established, you will receive an appointment confirmation letter in the mail.

For parents, legal guardians, etc.

We gladly accept self-referrals. A referral from a physician is not required. The instructions are the same as mentioned above. You can complete and submit our Patient Referral Form and/or call our referral department at 612-596-6105.
