To request copies of your medical records, please contact health information services at 215-430-4045.

Medical records are confidential. Therefore, we must confirm your identity before releasing a copy of the medical record. You can obtain the necessary authorization form while at Shriners Hospitals for Children in the medical records department, or you may download the Authorization for Disclosure of Health Information Form (English) or (Spanish). Federal HIPAA laws require an authorization form be completed for all medical record requests unless the medical provider is requesting them for care. Only the patient or his/her legal guardian can sign the form.

Requests for copies take 10 to 15 business days to process upon receipt of the authorization form. There may be a $1.46-per-page charge depending on the size of the request.

Insurance questions

At Shriners Hospitals for Children — Philadelphia there are no barriers to care. Doctors determine the course of care and all care is provided regardless of the families' ability to pay.

If the child is covered by private insurance or a state funding plan, the hospital will bill and accept reimbursement for services provided at our hospital. However, insurance coverage is not needed for care. Any child under 18 with a medical condition that is within the hospital’s scope of services is eligible for care.

Please have all insurance information such as primary, secondary or tertiary insurance coverage readily available at the time of scheduling an appointment and upon arrival of your confirmed appointment. However, lack of insurance coverage does not determine if care will be provided.
